We want to partner with you in creating a healthy, safe, and fun summer for your child.
Below you will find access to required camp forms in addition to camp handbooks and documents as they become available for the upcoming summer.
May 1 is the deadline to submit required camper forms, including:
- PHYSICIAN RECORD OF PHYSICAL EXAMINATION (REQUIRED)
- MEDICATION ADMINISTRATION FORM (IF APPLICABLE)
- ALLERGY ACTION PLAN (IF APPLICABLE)
- ASTHMA ACTION PLAN (IF APPLICABLE)
Because camper health and safety is our priority, please note that your child will be moved to a registration waitlist if we have not received required forms by May 1. Note that all health forms must be dated within 1 year from camp’s last day. Health specific questions may be directed to our health director at summernurse@polyprep.org.
Please note: If due to insurance reasons, your child does not have an appointment for an annual physical until after May 1, kindly submit last year’s physical exam with the understanding that your child cannot begin camp until a current exam has been approved by our health director.
